How to Register and Process

 

In order to register we need to ensure that you are in our catchment area. Please check this by entering your postcode below. If you fall out of the area please contact our reception team who will be able to discuss this further with you.

Once you have completed your registration form (GMS1) and a health questionnaire you will then need to provide identity proof such as a letter and one form of photographic evidence.

Once the documents are completed and accepted, you will be invited for a health check with a member of the healthcare team. Once this process is complete you will then be registered with the Practice.

All of our patients have a GP partner that they are assigned to and you can express a preference about the GP you are assigned to who when aged 75 and over is your accountable GP. You can express your preference during the registration process or by contacting the practice.

If you are not registered with a GP but need treatment at St Laurences Medical Centre, you will need to complete a temporary registration form.

The Reception Team will give you a Temporary services form GMS3 to complete or you can download the form here.

You can register as a temporary resident if you plan to live near the GP surgery for up to 3 months.

After 3 months you will have to apply to register with that surgery as a permanent resident.

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You'll remain registered with your permanent GP surgery.

St Laurences Medical Centre will pass details of any treatment you have to your permanent GP surgery. They will add the information to your medical records.

If your application to become a temporary patient is refused, you can still receive emergency treatment for up to 14 days.

 

What information do I need?

Try to have the following information with you for the first time:

  • details of any medical conditions you have
  • details of medical conditions you've had in the past
  • the name of any medicines you're currently taking
  • details of anything you're allergic to
  • contact details for your permanent or previous GP surgery
  • your NHS number

If you are registered at St Laurences Medical Centre, you can access some health services online.

You may be able to:

  • Contact your GP for advice and support
  • Order repeat prescriptions
  • See parts of your health record, including information about medicines, vaccinations and test results
  • See communications between your GP surgery and other services, such as hospitals
  • Book, check or cancel appointments with a GP, nurse or other healthcare professional
